Membership Forms

The Hampstead PTSA Membership welcomes Parents, Guardians, Teachers, Staff, Family, Alumni and PTSA HMS students! 

Now it's easier than ever to sign up with your credit card through PayPal*! After selecting your initial membership, there is an option to continue shopping that will allow you to sign up for additional memberships in one transaction. Once payment has been made please fill out the membership form and turn it in to your child's classroom or email a scanned copy to treasurer@hampsteadptsa.org.

If you prefer to pay by check or cash, please make the check payable to Hampstead PTSA. You will just need to fill out the membership form and turn it in along with your payment to your child's classroom. (Please put in an envelope marked "PTSA Membership.")
